The Community Collaborative for Health and Wellness (CCHW) is facilitated by Fallbrook Smiles Project and the Fallbrook Regional Health District.  It is comprised of representatives from various health and social services organizations and agencies of the Fallbrook and Bonsall Community. The CCHW meets once a month to not only share their programs and lend support to those of other organizations but also to identify, discuss, gain and/or give information and to initiate or recommend action on matters of concern to the health and well-being of the community.

The CCHW meets on the third Wednesday of each month (except December) from 10:30 am - 12:00 pm at the Fallbrook Regional Health District administration offices at 136 South Brandon Road in Fallbrook.
